Upon scheduling your first visit, you will be asked to fill out our patient intake forms which you will need to bring to your appointment. These forms can be emailed to you in a printable PDF format or they can be downloaded through our website. If you do not have access to the internet or a printer, please ask to have the forms mailed to you or arrange to arrive 20 minutes prior to your scheduled appointment time to allow for time to complete your paperwork.

Our office accepts many forms of health insurance including HMOS, PPO's, Medicare and TriCare. We encourage you to view our SERVICES page for a listing of most of the insurances that we accept. Co-payments, deductibles, charges for non-covered services are due and payable at time of service. We accept all major credit cards. There is a $25 charge for returned checks.
We offer cash discounts for our uninsured patients with full payment due at the time of service. Due to our insurance contracts, we are unable to discount our contracted rates or patient-allocated balances.

Our office is HIPAA (Health Insurance Portability and Accountability Act ) compliant and we have established a secure patient portal for all communications to and from our patients. After your first visit at our office, you will receive instructions on how to register for access to our patient portal. By establishing your unique log in and password, you will be able to request an appointment online, view office visit summaries, billing, lab results, and also communicate directly with your primary care physician and our office.
Our office is located within The Campus Professional Buildings just off La Jolla Village Drive at 8950 Villa La Jolla Drive, Suite C129, La Jolla, CA 92037.
Our medical office is one flight above the parking garage in building C on the 1st floor. There is an elevator and stairs. If you are in wheel chair or in need of assistance, please call our receptionist who can instruct you on the most convenient way for you to access our office. Our office space is ADA (American with Disabilities Act) compliant. |
Parking is complimentary with validation for up to two hours! To get free parking, just scan the QR code visible throughout the parking lot, enter your license plate number and the code: ljvfmg2. It's that simple! If you have any trouble, let our receptionist know and he or she can assist you.

A fee of $30.00 will be charged for office visits that are canceled with less than a 24 hour advance notice.

The standard processing time for all forms is 5 business days. If you need a form filled out more urgently, an additional expedited Fee of $25 will be charged for any request needed in less than 5 business days.
